Vial of Life Program

What is the Vial Of Life Program? Your CCMD HOA Board has approved your Security Committee to participate in this nationally recognized life-saving program. Vial Of Life decals and forms are available at the CCMD Community Center.

The Vial of Life or Vial of L.I.F.E. (Lifesaving Information For Emergencies) is a program saving countless lives each year by providing emergency responders (such as paramedics, firefighters, or police) with pertinent medical information when an emergency occurs, and you or your family member might not be responsive, or able to communicate. The responding official agencies will recognize the decal on your front door or window, and know where to find your information.

Five Life-Saving Steps

  1. Fill out a Vial of Life Program Information Sheet for each member of your family. Note, the form is front-and-back. Answer all pertinent questions.
  2. Fold and place the forms you filled out in a quart-size Ziploc plastic baggie. (Be certain to update the information as your medications and/or medical information changes.)
  3. Place the 1st decal on front of the plastic baggie, and the 2nd decal on left-hand front of the refrigerator at eye level.
  4. Affix the Vial Of Life plastic baggie on the left-hand side of the refrigerator at eye level by using a magnet clip or a tape.
  5. Place the 3rd decal on your front door or window so it can easily be seen by anyone responding to a medical emergency.

Please tell your neighbors about the Vial Of Life Program. You might be saving their lives in a medical emergency. Remember that the responding emergency or medical teams need the information in the Vial Of Life to save YOUR life!
